Yes, there is not a warning issued for disruption, but there is a lockout for it. This was one of those situations where someone was found doing something they really shouldn't and bypassed the whole in-game warning system. It's not common, but it does happen.

I can't say what it was they did. That would be giving out specific information that could be tied back to a specific person too easily and while it might not violate the letter of the privacy rules, it would give the spirit of them a big kick in the face. So no comment on specifically what.
I can say, there is absolutely no way this could be done by mistake in any way shape or form. It was deliberate, intentional, and malicious, and the odds of someone doing it by mistake are about the same as the odds of someone Macgyvering a Saturn V together in their backyard out of a swingset, a fire extiguisher and a couple of garbage bags, and surviving a trip to the moon and back.

Okay, since when did The A-Team have more recognition than Mac-the-god-of-all-things-Gyver?!? The A-Team was a bunch of wussies in a bongo circle compared to MacGyver.
