Trying to figure out how I died. Not Bleeding and had 11% health then I just died.

Was there a bard in the area performing Abandoned Heart, by any wild chance? Or were you perhaps holding onto a grave item? Though you probably thought of that already.

I thought that when your character falls unconscious from vit damage, you get some kind of [very minor] health boost. Maybe that boost wore off before you gained enough normal health back?

As I recall, when you drop into dying it converts fatigue to vit and allows you to start to recover, however once the dying mode wears off you pay back the 'free' health and if you haven't recovered enough, you die. More stamina will let you recover more often (due to faster recovery of Vit/Fatigue), as well as a Bard Lilting, Empathic assistance, or even a Zephyr can make the difference.
