There I am, minding my own business when a friend comes along, she has a few scratches, so I heal her, and she gives me a gem. Couple scratches, one gem, no problem
Another person in the room whispers, of course, asking for a healing, body almost completely destroyed, I heal them, and what do I get for my 20-30 mintues of healing myself? ONE gem!
Luckly the next person tipped a gold for 3 scratches.
I'm seriously going to start charging up front, as I also got stiffed for curing a disease earlier today.

Something I've found goes a long way...
If they offer you a small gem- Topaz, crystal, or diopside- decline and tell them to keep it. After all, if that's all they feel they can afford to offer, they need it more than you do. (and this way you keep that guise of the kind, generous empath. Heh. Not that I have any sort of guise.)
More often than not I've found it makes them feel rather silly and offer something better instead. Mind, I only do this to people whom I have strong suspicions or know for a fact that they can afford to offer more.

When this happens to me, I take out my katar and thrust it towards their heart.
Of course, I just get shocked and stunned and fall over and lay around for a while. But it gives me time to figure out which bar in which inn I'm going to visit in the evening.

I once healed someone who was very wounded and then they walked away. I was pretty upset that I didn't get a thank you after the amount of wounds I healed. So.. I went and found him in blood wolves and thumped him and said "You should thank those who heal you. You may need them to save your life once day." Then I went back to where I'd been sitting. He came back a few minutes later and whispered, "I did thank you for the healing!" <OOPS!> I scrolled back and saw that he did, in fact, say thank you while I was busy transfering all the wounds. I appologized for the thump and he gave me a tip since he hadn't before. Talk about putting your foot in your mouth.
One way I get tips since I am a very drunk empath is..
"I'm too sober to heal right now. I either need some booze or some booze money first."
Not everyone carried booze around with them so sometimes I get booze and sometimes I get money. I then promptly go to the free bar in Lang and get some booze, guzzle it, then heal them. If they mention that its free booze I simply say, "Gotta tip my waitress." Most people now do carry booze with them and give it to me often along with a tip which works out fine. Some people stop by just to give me booze or send their fams to drop it off to me. I never run out of booze unless I'm doing a lot of healing then I drink a lot more. Booze dulls the pain!
While I do get tips this way, I don't do it for the money. I do it for the sake of roleplaying. Besides.. if they get mad about it thats one less person I have to spend my time healing.

I would not turn down a gem if someone offered it to my empath. For as my Warrior Mage does, small gems are pretty much his currency. He's backtraining LC and sand sprites are bedecked with small gems. It is often his way of tipping is offering gems, not just one or two, I have a script which goes through every gem type and offers it to the empath. Usually they start declining after the fifth or sixth gem. If I am out of gems, which is a rarity I will transfer money with a minimum of 1 gold tip. I mention this because I've had a couple of empaths decline my gems and just nod... hope they were not offended by the start of my payment.

The thought is kind but please remember gem pouches are your friend. Empathing is more typing intensive than any other guild I've played -- your empath is healing him/herself while 10-12 gems are changing hands and being stowed. Maybe the wounds you lost aren't much by themselves but in combination with whatever the empath had to begin with and the healer might well need those keystrokes to stay alive.

Can't speak for other empaths, but I know mine rarely turns down gems. However, if I understood the original post correctly, it's not the fact that small gems are offered, it's that only ONE gem (or perhaps two) was offered for a fairly substantial healing situation, and that the patient thought nothing of requesting two fairly expensive herbs in lieu of healing. The latter struck me as funny because the patient seemed to think it was okay to ask the empath to give up herbs worth several silver, but offered a gem worth only a couple of bronze. I would think if you have the gumption to ask someone for herbs worth several silver, you ought to be willing to tip the empath at least that much for his/her services.
What few hunters seem to remember is:
(A) Giving us wounds is no longer the ONLY way to work empathy, so you're not doing us some huge favor anymore, so therefore we don't look at the opportunity to get wounds as some special bonus for us anymore;
(B) Gems and money aren't the only things we take. No we don't want your rag halfling doll, but a few scrape-able pelts would be terrific, even if they aren't worth THAT much cuz we can learn mech lore and skinning off them;
(C) We have to heal four wounds on ourselves for every one we take from you, so make it worth our while and tip a little more.
My Ranger's rule of thumb is to try to estimate my haul for the hunt I just completed and give at least 20-30% to the empath who heals me. If I haven't hunted, I'll give a minimum of 4-5 small/medium sized gems for a basic "clea-up" (hate that term). Obviously, younger players can't give as much and older players can afford to give more. Just don't be stingy if you don't have to be.

I always refuse to accept topazes. I am not the only empath who refuses to take certain types of gems. Usually I will tell the person offering me a topaz that if that is all they can offer me they need it more than I do. Somehow, the bigger looking hunters tend to interpret this as "I don't want a tip" though. Heh.
Just figured I'd throw that in...If an empath refuses a gem, you might want to ask them why if you would really like to give them something.
Also..As a point of reference, I tip in golds on my main hunter and rarely ever gems. I only ever seem to get small gems (like topazes) in rock trolls and I suppose it's carries over from playing an empath for so long. I just think it's insulting to hand an empath who just healed me a topaz or crystal unless it's a pouch full of them...Especially because I wear a lot of nice stuff, and I know personally how frustrating it can be to get a tiny gem from somebody with a couple hundred plats worth of fluff.

If you're honestly worried about it, follow Passionata's advice. Even an empath who might turn down "small" gems is unlikely to turn down a gem pouch.

I usually tip boxes and such, but when I'm out I just tip several gems, one at a time. So if the first one's refused, then I really won't offer any more. So that's several gems that are lost because you thought the first one was the only one being offered. Just something to think about.

This is the main reason I'll accept the small cheap gems, even though I do find myself thinking, "Wouldn't a gem pouch be easier on everyone?"
However, I will decline all monetary tips of less than a silver. Really, if all you can afford is that 37 copper dokoras you had at the bottom of your pocket, you certainly need it more than me, mister Marshal, sir.
My main hunter (a mid-level Cleric) will ask Empaths what they want, since she usually has at least the following: money, gems, or boxes. If she's hunting something she can skin, that's also an option. If they say boxes, she tries to give them at least 5 (generally rock-troll level or a bit harder) (and usually they start refusing! stop refusing her tips!), if they say skins, she hands over the whole bundle, if they say gems or money, she gives them about a gold or the equivalent, gems neatly pouched. :)

I don't mean this for you, hon, but for anyone else who might be reading. The way I see if, I don't accept a tip, it's no real loss to me, but if someone gives me a chinzy tip, they are always and forever "The chinzy tip giver" and I think much less of them for that.
As per her heritage and custom, my char will refuse the first offer or two, but will often accept if the person insists or says anything along the lines of "I want you to have this" etc. But still will not accept flowers, chocolates or other romantic gifts from unfamilar men because, again, her mother brought her up better than that.
Anyhow, I think being handed off a bunch of gems is a bit tacky, anyhow. I prefer them in boxes, especially if that's where they came from...or as coin. Won't take a tip, but if someone just gives the coin, that's good.

My barbarian likes to tip with gem pouches when he has them. Ironically, it seems to send the empath that healed him into shock after they accept it and they usually try to give it back. Something about half a dozen abrasions not being worth two dozen small and medium gems. <shrug> If I don't tip with a gem pouch I'll tip a gold minimum. More depending on how much I have.

Don't get empaths started talking about chintzy tippers unless you're ready to listen for a long time ;)
Farman definitely believes in accepting tips since doing infirmary duty is a job to him. Gems of any size are a good tip in his book. Small ones are better than no tip at all he reasons, and he especially likes high-value gems that he can add to his app pouch. Boxes are happily accepted, since they teach two survival skills if he can open them himself, and contain a nice chunk of treasure if he can't. He will respectfully decline skins however, since he doesn't like handling parts of dead animals as it brings to mind wounds far too extensive to heal. There's nothing wrong with cash, as well ;)
He usually does pretty well with tips overall since the high tippers cancel out the low ones. But two patients from the last few weeks stick out for giving exceptionally low tips relative to their personal wealth. The first was a Very Very Very Old And Well-Known Forger who gave him a handful of small gems with total worth under 200 kronar for taking a lot of wounds off his body, and the second was a Well-Known High-Level Trader who waved at him and said "I have to run, I'll tip you later", which of course he never did after having crossed Farman's path several times since then.
This last excuse really gets Farman's goat--the bank is a short walk away from the Crossing Guild and something's wrong with you if you can't spare 20 seconds to run down and grab some coin to tip your healer.

Maybe you're special or maybe I just have higher standards...But unless somebody comes along and dumps 3-5 gold in my lap to make up for the last 10 people who gave me under 2 silver gems, I don't really consider it cancelled out. It's one of the reasons I don't heal often- People just don't offer me enough to make it worthwhile for me to heal them, do the whole social hello-thank-you-you're welcome-goodbye-be-well song and dance, and spend the next 30 minutes to an hour healing myself.
All I can say is...Thank god for perc health.

Well, it becomes an even lamer excuse in Riverhaven. Bank is...what...three or four blocks south?
<cackles>
There have been times when people have said to Syrath "I'd tip you but I don't have anything on me."
And she'll say something like "Oh, that is alright, the bank is not that far."
::chuckles:: They get so surprised.
"I'm sorry, but I just sold all my gems."
..."Oh, that is alright, I'll accept coins."
"I don't have anything on me now, but I'll pay you later." or "I'll be right back with your tip" ..and they never come back.
Generally I do try and give the benefit of the doubt. I mean, just as we are playing a game, and just as we heal hundreds of people, they have been healed hundreds of times. So, they could have just forgotten, or suddenly had a person that needed dragging or help.. You never know.
Not that I'm saying there aren't people out there who use "I'll be right back" as a way to get out of paying Empaths, because there are.
And even with the people I give the benefit of the doubt, at times..I'll approach them later on. I do try and put it as nicely as possible. "Is this a more convenient time for you to pay me for before?"
Those people who use that excuse are even funnier when.. They get healed, they say they will be right back to pay you, they don't come back.. but later on that same day they come back and ask you to heal them again..no acknowledgement of the previous payment.
Sometimes I'll ask them "What happened to you before? I waited around for you to come back with my payment but you never showed."
Yeah, look at the beautiful, beautiful people.

I don't use blacklists or nothing(yet), but people who are ratty enough to stick out in my mind get left to die.
Personally, if you want to make money healing, don't do blacklists, do (positive color)lists. I find healing people I know to be appreciative of the awful suffering of healing(Trust me, I know. I nearly cut the tip of my finger off last night trying to take apart my food processor. Deep slashes and chunks missing is NOT dandy to have) has two good effects:
1) Since I'm selective in my healing, I'm never overloaded on wounds. I'm always available to help those who I choose.
2) I'm never dissapointed/angry/depressed over some slime bag leaving me limbless and poor like I was some sort of parapalegic panhandler.

Yes, but you must first heal a person who is unknown to know that they will give you a good tip. My blacklist is well, black. In the Wizard FE with the black background all I see is.
says "Can I get healed, I'm about to die"
is in shock and is dying!
............ <time passes>
just died in front of you!
You say "Oops"
You throw three colorful juggling balls in the air,.....
I've actually had a few people come back after departing and ask my why I didn't help them, I told them, and most of them appologized and a couple even gave me a backdated tip!

When we think it's rude, of course, to reveal the names of those we feel tipped badly or if we insist on giving back tips that are, what we think, too generous, we allow the problem of low tips to persist. No one does it to us. We're not victims. We have the power to change it if we really don't like it.
