Has <something happened> with either of these systems recently?
I have seen just a ridiculous number of open-high rolls making things effective against me (some where the creatures only have a 5-8% chance of effectiveness... but they get an open result of 191 and it hurts, a lot).
Anecdotally, this might have begun at about the same time that I started seeing "-3892" results of the re-written ogres against my capped character, so maybe something unintentional got changed at that same time? (And seriously, why does a maneuver attack THAT bad not just break their neck in embarrassment?)

And likewise a pretty silly number of "armor failures following / followed by / mixed in with 1==FUMBLE" results. I mean, I've only got a 2% chance of armor failure, and consecutive results? Really?
I wouldn't complain nearly so much if one of the characters wasn't wearing some of the Karma armor, unlocked to 4% chance of absorption in all seven (7) different possible damage types, and not only do I seldom see it go off when I am hit, I certainly NEVER see it go off on consecutive hits...
...and it should, notionally, be at least twice as likely to happen as 2% spell hindrance. <snort>
