Out of curiosity, when a cool item gets put into the treasure hopper does it go out in all three instances, or just one instance? In other words, if a GM makes a super cool sword and puts it in the treasure hopper, does it just go out in Prime? Do items automatically get added to all instances, or is it manual?

Yeah, I tend to let some of my favorite VG stuff leak into the game in my own way. You'll find Final Fantasy (mostly 4 and 6), Phantasy Star, Shining Force, Lunar, Castlevania, and Lufia references in a lot of the areas I paint, or merchants I make.
