Will choosing a last name have to take place at the clerk, or can you do it anywhere at any time?

At the clerk. Every option costs plats, with reserving your name so that others who come after you that want it can't have it costing 10 plat. The options, if I recall, range in price from that all the way down to 3-5 plat or so.
Giving your name away to anyone else requires a trader of sufficient level, which in Plat has been pegged at 25th circle (bet it's 50th in Prime, but nobody's said yet).

Out of curiosity, where did you hear this? This is part of Phase 2, which hasn't been released yet anywhere. Is there another GM post saying that choosing a name will be a clerk function? Because honestly, walking around town I can introduce myself as anything I want. I find it rediculous that they're charging plats just to have a last name. Lots of people don't have plats to spend on a last name. I was also wondering if they're going to start allowing people to choose last names in the CM again, but forgot in my first post. Any news on that front?

Phase 2 is released in Plat. And yours would be the first question about the CM, actually. In Plat, last names are still not available via the CM so I imagine we're down to two scenarios:
1)GMs want to allow us to roll up with last names for free (more likely to be tacked onto our debt), but the CM code needs to be handled separately (same type of issue as with spellbooks and bonded items)
2)GMs don't want us to roll up with last names, forcing us to pay 10 plat to the clerk each time we want one

Phase 2 is in Plat and it seems to be pretty straightforward. I'm not sure about the character manager, myself. It is a good question to find the answer for. I imagine that it won't be updated for last names (if it is going to) until the features rewrite is done and implemented.

Incorrect (as is usual in a j'lo post) It is only 10 plat if you want to register a last name for your exclusive use. In Plat, it is only 1 plat to choose a last name if you haven't got one.
This is what the clerk says when you ask him about that option:

The clerk says, "If you'd like to choose a new surname, ask me for CHOOSE NAME (ASK CLERK FOR CHOOSE <name>), where NAME is the surname you wish to possess. However, be warned that I won't tolerate any mischief. If the last name sounds acceptable, I'll have the clerks research it for any claims. Assuming there are no issues, you'll be granted the last name. If I've found that you have chosen a last name that is OBVIOUSLY unacceptable, not only will we revoke the last name, but you will not get a refund either. The cost to choose a new surname and for it to be researched is 10000 Kronars."
Not sure if the Prime price will be different.
