How does one go about getting the affiliation title corresponding to one's sect? I've seen people in game with them and was wondering if they appear in the affiliation title list upon reaching a certain skill requirement or circle, or if they have to be obtained from the sect leader. Any insight would be much appreciated.

The "new" titles (as in the ones covered in the update I did last year) are earned like any other title. You meet the requirements, and it'll be on your list.
The affiliation titles you need to get from your sect leader. I believe the requirements for that are still technically PAFO, so I'm going to play it safe and leave it at that.
