I was finishing up a 2 hour hunt in crocs when...
With the speed and temerity of a swooping falcon, you sweep a bastard sword at a blue-belly crocodile. A blue-belly crocodile attempts to evade. The sword lands a good strike to the crocodile's chest.
[You're incredibly balanced and in very strong position.]
[Roundtime 6 sec.]

Then nothing...more than 3 minutes of nothing so I tried to get to the boards. Couldn't get to the boards either.
In a panic, I tried to reconnect. Never made it all the way through.
Two minutes later, I tried to reconnect again. No dice.
Two minutes later, I went to try to reconnect again.
Two minutes later, when I hadn't made it through I tried again. Just as I hit the key for the reconnect, I apparently made it in to see 4 crocs rush in on me.
Since I had not seen the reconnect and was in the middle of connecting again, I didn't have a chance to stop and fight. I had barely a scratch on me when all this started.
Less than 10 secs between that and the login.
On this login I was dead, no crocs, no recourse, and the weapon that had been in my hand when all of this started was now in my weapons belt. I could also look in my weapons belt while dead which was a bit odd.
I ended up departing full and yes, lost 2 hours worth of exp.
So general rule of thumb questions...
How long do you wait for the game to catch up before you try a reconnect? Is there a better way than that to handle it?
How did the weapon end up back in the belt?
Does reconnecting trigger an insta-death?

I try to reconnect ASAP...and keep attempting, even if I can't get in. If the problem is with your ISP connection and not DR, there is an ugly process called ghosting where your character is in the game, but is not connected to the outside world. In combat, this is generally not a good thing.
The important thing is wait each time until you KNOW you are not connecting (30 seconds?). If you log in and out repeatedly when you are engaged, you can trigger the combat logging penalty which is even a WORSE thing than ghosting.
Is there a better way than that to handle it?
Nope. Though I will say if you suspect your ISP is the issue: log in and get out of the hunting area ASAP...then do something 100% safe until you're convinced the connection is rock solid.
How did the weapon end up back in the belt?
Either there was a time warp (game crashed and they had to reset to some previous point in time) OR you may have triggered the combat logging penalty. I'm not really sure on the penalty, because I don't log out once engaged in combat...though like you, I've had my share of aggravating disconnects...I lost a kertig sword due to ghosting.
I HAVE seen the combat logging penalty on another player, he logged in with a whimper and four critters immediately rushed him (instant melee). I was dragging hamburger to the empath guild. As you mentioned "being rushed," it sounds like you got hit with the penalty.
Does reconnecting trigger an insta-death?
Not always. I've been in rooms where there's a quick 3-4 per hunter gen rate, I was dancing with 2, but three should have pounded me down pretty quick...but when I logged in from an ISP drop, injuries have ranged from no injuries to mild/moderate injuries to dead.
Bottom line, given connectivity issues or DR crashes, I'll take a DR crash every time because GMs attempt to right many wrongs (they've got ways to insta-rezz dead folks, reset caravans, etc). Your ISP provider could give a darn less that you lost your custom +99 sword o'doom. Unfortunately, DR GMs can't replace the sword o'doom either, unless it's been pre-registered.
Hope that helps.

The thing is that not 30 seconds before she went boom, Faih had said something to me via Chatter that she was making her way to my shop. So she couldn't have Discoed for very long, and I didn't see any signs of a time warp.

I know there was about two minutes between at least two of the attempts because I logged the connect times.
Part of what gets me is that nothing else lagged. IM's were fine. Rest of the internet was fine. It was just my connection with SIMU that got lost somewhere and it was both boards and the connection to the game at the same time. I tend to check those things because if it's everything I assume it's my connection.
I must have triggered the penalty which stinks since I wasn't running or logging. Guess it doesn't differentiate between log out and reconnect. This makes me think that when it happens, only do one reconnect attempt. If that doesn't work, wait 10 or 15 mins and try again. I can't help but think that my multiple reconnect attempts made the penalty worse.
I dunno.

If you can get to the main page, http://www.play.net/dr, you can logout of the game that way so at least you know you aren't ghosting in combat. I have no idea how you kept your weapon, though.

This is a good tip.
