Okay. I assisted about this IG because it seemed pretty bogus to me, and the GH who handled the assist suggested to post here..
So I die in boas in throne, depart, and run back to my grave, but a room before I get to my grave, this happens..

Patches of elephant grass and clumps of dagger-like reeds part just enough to reveal easy access to the inlet shallows.
Obvious paths: northwest.
You can't go there.

Your body feels in extremely bad shape!
Your spirit feels lifeless.
You are extremely tired.
You have a few nearly invisible scars along the neck, a few nearly invisible scars along the right arm, a few nearly invisible scars along the left arm, a few nearly invisible scars along the right leg, a few nearly invisible scars along the left leg, a few nearly invisible scars along the left hand, some tiny scars across the chest, a few nearly invisible scars along the abdomen, some tiny scars across the left eye.
You have no significant injuries.

You feel like you're about to die!!
You feel yourself falling...
You feel like you're dying!
You feel like you're about to die!!
Your death cry echoes in your brain as it quickly dawns on you that you have just died! Already, you feel the tug of eternity upon your spirit and you struggle to remain tied to this world.
I'm assuming this is because of the leeches in the area, although I didn't think that they gave an instant vitality hit, from prior experience with them in that area. Also, I thought that when they do give a vitality hit, that you get messaging along the lines of 'you feel tired' etc etc. Can anyone shed some light on this please? Thanks in advance.

From my experience with leeches (from the Muspar'i sand pit) you do take a vit hit when they get you. Usually you get the tired message after a pulse or two not always from the start.
Icky thing to happen, but you had a vit that was pretty close to '0' after departing so it makes sense a little leech would do you in.

After having alot of extensive experience with leeches...like I think many Empaths have.. I agree.
Basically when you die you have a Vit loss of 99%, and a high loss of Fatigue which is why if you die in the Crossing you can't climb up the stairs immediately. The reason Leeches are so nasty is because Leeches give you a Vit and Fatigue hit before you can even see them, its like a way of knowing you got it. In your case, it was fatal since you were still in pre-death vunerablilty.
Also, as far as the messaging, I remember clearly that with leeches you don't get that messaging untill they've sucked enough out of you. In your case, you were already pretty bad so it just lead to instant death. Even if you had the messaging, by that point it would have not made a difference. And I believe the feel tired is a Fatigue hit, not a vitality hit. Well, live and learn I guess eh?

>Thanks for the input guys, it explains a lot. Bit of a crappy place to have the leeches, especially without RT moving from room to room, but oh well. Also, is there a perception check on noticing the albino leeches, when you check your health?
I don't know about the albinos, never been there, but I know the red and black leeches as well as the bloodworms were susceptible to this little thing I figured out:
>peer leech
If you got the messaging:
"[Usage: PEER (direction|player|creature|object)]"
You had no leeches, but if you got the messaging:
"What do you want to peer at?"
It meant you had a leech you just couldn't see by checking health yet.
